About Us

Who are we?

A multi-disciplinary team at the University of Rochester in Rochester, NY. Our investigators include Dr. Jinjiao Wang (nurse scientist), Dr. Jenny Shen (hospitalist), Dr. Kobi Nathan (clinical pharmacist), Dr. Yeates Conwell (geriatric psychiatrist), Dr. Thomas V. Caprio (home health director and primary care physician), and Dr. Ying Xue (nurse scientist).

What is the goal of this study?

To develop an intervention to optimize medication use and deprescribe when possible for recently hospitalized patients who are receiving post-acute home health care visits.
